As an added convenience, particular fields, such as name and address, can be completed on your computer screen and then printed. National H.O.G. guidelines restricts any alterations to these forms. Please do not make any changes to these forms, other then adding your information to the editable fields. 1. Chapter Membership Enrollment/Renewal and Release Form
   Use this form when joining or renewing your membership with Rose City H.O.G chapter #4105. National H.O.G. requires all members to renew their local chapter membership annually.
2. Chapter Event Release Form - Adults
   Use this form when attending Rose City H.O.G chapter #4105 rides and/or events. All attendees (adult) must sign.
3. Chapter Event Release Form - Minors
   Use this form for minors attending Rose City H.O.G chapter #4105 rides and/or events. Must be signed by parent or guardian.
4. Minor's Assumption of Risk Acknowledgement
   Use this form for for minors age 12 and older attending Rose City H.O.G chapter #4105 rides and/or events. Must be signed along with Chapter Release Form for Minors.
5. Group Riding Class Enrollment Form
   Use this form to enroll in Rose City H.O.G chapter #4105 group riding class. Class dates and enrollment instructions included on form. Must be current Rose City H.O.G. member.
